1. What you need based on your hearing loss
2. What you want based on your hearing situations
If either of these categories are ignored during the selection process the results will be less than optimal and sometimes disastrous.

Fill it out and you can either:
1. Call our office for an appointment to discuss your results
2. Fax or mail the form to us for review
3. Submit the form on line after you are finished so we can review
your comments and then begin to guide you through the selection
process.
